How to Fill in Form 6/FCAF
One form to rule them all!

If you’re teaching DP Film, this one’s non-negotiable. The 6/FCAF (Coursework Authentication Form) is something we need to complete for every single student. It’s our way of confirming that we’ve supervised, discussed, and authenticated their coursework across the different assessments. It also assures the IB that what the student submits is, well, actually theirs — especially important in these ChatGPT-powered days.
Here’s a clear and easy guide to help you fill it out — no stress.
What is the Form 6/FCAF?
It’s a required form for all four IB Film assessment tasks:
- Textual Analysis (TA)
- Comparative Study (CS)
- Film Portfolio (FP)
- Collaborative Film Project (CFP) – HL students only
You’ll need one form per student, and you must include a short teacher's comment for each task. This comment should explain:
- What kind of support you gave
- How you checked that the work is authentic
What Goes in Each Section?
🔹 School Name and Code
Write the full school name and IB code. If you don’t know your school code, your IB Coordinator will have it.
🔹 Candidate Name, Code, and Number
Again, get this info from your IB Coordinator — and double-check that it matches what’s on IBIS.
- The code is usually a mix of letters and numbers (e.g. xyz123).
- The candidate number is typically four digits (e.g. 0047).
What to Write in the Comment Boxes
For each assessment task, you’ll write a short paragraph (2–4 sentences).
It doesn’t have to be long or fancy — just enough to explain:
- How and when you met with the student
- What kind of support/discussions you had
- How you made sure the final work was their own
Sample Comments You Can Use (or Tweak)
Textual Analysis (TA)
I met with the student during the early planning stages and again after their first draft. We discussed extract selection, film elements, and relevant cultural context. My feedback focused on structure and technical vocabulary. Final draft reflects their own work and follows IB expectations for academic honesty.
Comparative Study (CS)
We discussed their film focus early on and then their choice of films. I reviewed a rough outline and provided oral feedback about balancing visual/spoken content. The student created their final video independently and cited all sources appropriately.
Film Portfolio (FP)
Met regularly throughout the project — from early idea development through post-production. The student shaped their creative intention and refined it with my guidance. Portfolio drafts were reviewed, and all work (film + documentation) was independently produced by the student.
Collaborative Film Project (CFP) (HL only)
The student worked as [insert role, e.g. screenwriter] in a group of three. I observed their contribution across pre-production, filming, and post. They took ownership of their role and showed growth in collaboration and execution. The final report is their own reflection and documentation.
Final Teacher Checklist
Before uploading the student’s work and this form, make sure:
- All relevant assessment boxes are filled in (depending on SL or HL)
- You met regularly with the student during the process
- Your comment is clear, specific, and matches what’s in the guide
- All sources and references are cited in the student’s work
